Dr. Milton Padgett is a certified civil surgeon with all appropriate designations to conduct immigration medical exams. Our experienced providers can evaluate immunization requirements and records and complete federally required forms, including Form I-693.

Medical Exam & I-693 Form

At Padgett Medical Care, we understand the importance of a thorough and accurate immigration medical examination. Dr. Milton Padgett is a certified Civil Surgeon with years of experience in conducting these examinations.
A medical exam is necessary to ensure that you are healthy enough to be eligible to adjust your status to admissible to the United States on public health grounds in order to apply for a US Green card. The doctor will complete a comprehensive examination that will include a review of your medical history and a physical examination. You will be sent to a specialty laboratory for blood work. Laboratory test results usually return within 48-72 hours. If the tuberculosis test is positive, a chest x-ray is required, which can add days to the process. Green card medical examinations are generally valid indefinitely.
After the physical exam, reviewing the immunizations records, all lab results and any additional vaccines records are received, the doctor will complete Form I-693, once sealed by the civil surgeon it must not be opened by the applicant or anyone else, but for you to submit to USCIS. You will get a copy of the completed Form I-693 for your personal records.

What Is Needed
